Is it even possible? If someone tells you “botulinum toxin lasts forever” – don’t buy into that clickbait article.

The effects of botulinum toxin and filler procedures are temporary.
Their longevity depends on many factors, such as:

1.  The Size and Strength of the Facial Muscles (Area Treated)
In general, the larger and stronger the facial muscles/expressions, the quicker the effects of botulinum toxin and fillers would go away.

2. 
Individual Metabolism Rate and Other Lifestyle Factors
Similarly, individuals with higher metabolism rates tend to notice that botulinum toxin and fillers wear off faster.

3. 
Type of Fillers / Botulinum Toxin Used
The material of fillers can differ according to the type of material used, how cross-linked it is, and botulinum toxin also comes in slightly different variations.

4.   Possible Immune Responses to Proteins That Are Present in Botulinum Toxin or Fillers.
Immune responses are rare, but if present drastically reduce the duration of botulinum toxin and fillers.

All these factors are beyond our direct control, but there are certainly ways to assist the rejuvenating effects of botulinum toxin and fillers to continue to look more youthful.

To prolong the effects of botulinum toxin and fillers, you would essentially need to improve the condition of your skin. It is all about prevention and rejuvenation. The following tips do not directly prolong results of treatments, but it extends the time in between each procedure.

What about skincare products that claim to help replicate or mimic the effects of botulinum toxin / fillers?

There are no topical products available that can 100% replace botulinum toxin or fillers. You may have seen videos online on products that claim to give you the botulinum toxin or filler effect within an hour. These are temporary, and often at a higher cost. They contain high amounts of peptides, which temporarily smooths the skin and wrinkles, but do not work like botulinum toxin, which is administered directly to the muscle. Once the peptides are absorbed, the results go away as well. 

Certain ingredients such as peptides, specifically hexapeptides, may help reduce the appearance of light wrinkles – however, don’t count on it to replace botulinum toxin, as it does not hinder muscle movement. To quote Paula’s Choice: ‘Other studies on topical use of a moisturiser containing a 10% concentration of acetyl hexapeptide-8 have shown it does not penetrate beyond the uppermost layers of skin, even after several hours. That means it cannot affect factors in the underlying layers that lead to the formation of expression lines. Still, like all peptides, acetyl hexapeptide-8 has water-binding properties and skin-restoring ability. It’s not a throwaway ingredient, but neither is it as miraculous as the manufacturer would lead you to believe.‘

In conclusion, “botulinum toxin lasts forever” is… a lie. However, a great skincare routine can help to delay the ageing process and reduce the need and frequency of such procedures.

Can Botulinum Toxin Lasts Forever? | Mizu
Botulinum toxin is a neurotoxin made from the bacterium Clostridium botulinum. It is used in the management of wrinkles and for reducing muscle bulk for a slimmer, v-shaped face and jawline at aesthetic clinics in Singapore.

How Does Botulinum Toxin Work?

Botulinum toxin works by inhibiting muscle movement. To be exact, it prevents nerve impulses from being transmitted from the brain to the muscles. These impulses travel through our nervous system and crosses from nerve to nerve at junctions called the nerve synapses. 

Botulinum toxin targets and blocks nerve synapses, therefore breaking the message chain. Without this message from the brain to the muscle (or any other body part), the muscle will not receive the instructions to move.

More toxin = more movement inhibited (more nerve synapses blocked) = more of a “frozen” look.

For lines and wrinkles (such as the ones on the face and forehead), we usually start with small doses to help prevent dynamic lines from forming. 

For facial slimming / jaw muscles, it works by first preventing movement, then eventually the masseter muscle shrinks with the lack of usage and this leads to a sharper, smaller face! 

Botulinum Toxin Can Also Be Used on and for…

Microtox

The botulinum toxin works by blocking the nerve impulses to the sebaceous glands and sweat glands on the skin, which reduces oil production and sweat from the face. This in turn reduces clogged pores and minimises pore size. It also has the additional effect of preventing acne, brightening the skin, and improving skin texture!

  • How soon can you see results for microtox:  3-5 days to work.
  • How long does microtox last:  6 months
  • Units of botulinum toxin required for the whole face:  30-40 units for the whole face. Some combine it with hyaluronic acid (skin boosters).

Underarms and Palms to Prevent Excessive Sweating


Hyperhidrosis, or sweaty palms and underarms, can be treated effectively with botulinum toxin. Botulinum toxin reduces sweating by blocking the nerve impulses to the sweat glands in our skin. Inhibited sweat glands significantly reduce the amount of perspiration.

How soon can you see results for botulinum toxin to treat excessive sweating: 3-5 days to work
How long does botulinum toxin for excessive sweating last: 6 months
Units of botulinum toxin required to treat excessive sweating: Depending on the area treated – the larger the area, the more required as there are more sweat glands. On average, about 40 – 50 units for each arm or palm.

Note: Botulinum toxin administered for excessive sweating does not affect muscle movement as administered into the skin layer instead of the muscles. However, it is again important to go to an experienced doctor as the skin layer and muscle layer are very close together and incorrect technique can lead to unwanted side effects.

Calf Muscle Reduction

Calf muscle reduction is a popular treatment in Korea and a growing request in Singapore. When done on overly muscular calves, it helps to achieves a more feminine curve, making legs look slimmer and longer.

Calf muscle reduction works exactly the same way as how botulinum toxin works for jaw muscle reduction. Calf muscle activity is reduced and restricted, and calf muscles becomes weaker and smaller over the next few weeks. It is akin to how a bodybuilder loses muscle mass after some time if he stops training his muscles.

  • How soon can you see results for calf muscle reduction: 4 weeks as it takes time for muscle size to reduce once the botulinum toxin is administered.
  • How long can calf muscle reduction last:  6 – 8 months
  • Units required for calf muscle reduction:  More as calf muscles are bigger – an average of 75 – 100 units per calf muscle.

Note: Too much botulinum injected into the wrong areas in the calves can result in over weakening and one may experience weakness even when walking.

Shoulder Contouring

Botulinum injections for shoulder contouring aims to achieve a lean, elegant, and more angular shoulder line, especially for women wearing tubes or strappy dresses. Due to over usage of mobile phone (text neck) and long office working hours, some people can develop a poor posture, which leads to overdeveloped trapezius muscles – masculine, rounded shoulders. 

Like calf and jaw muscle reduction, shoulder contouring aims to reduce the size and bulkiness of the trapezius muscle. The trapezius muscle is a muscle that connects the neck and the shoulders and is often the area that feels stiff due to poor posture. For those suffering from stiff and aching shoulders, shoulder contouring has the additional effect of removing these aches! 

  • How soon can you see results for shoulder contouring:  3-4 weeks for muscles to shrink.
  • How long does shoulder contouring last:  6 – 8 months
  • Units required for shoulder contouring:  An estimated 30 – 50 units per side of the trapezius muscle.

 

Gummy Smile Reduction

Showing too much gum when you smile? Yes, a bit of botulinum toxin (and sometimes lip fillers along with this) can change your smile! 

A gummy smile happens for some individuals because of the overactive muscles that pull up our upper lip when we smile, causing your upper lip to look thinner than it is and therefore overexposing the upper gums.

Botulinum toxin works by relaxing and reducing the movement of the upper lip muscles. Once these muscles are relaxed, the upward pull of the lips is reduced, and less of the upper gums are exposed.

  • How soon can you see results for gummy smile reduction:  Results are quick, usually within 3-5 days, as the relaxing effects of botulinum toxin sets in.
  • How long does it lasts:  4 – 6 months, as the lips are often used.
  • Dosage required for gummy smile reduction:  Only a small amount of botulinum toxin is required, between 5 – 10 units.


Note:
 Too much botulinum toxin administered to the smiling muscles can result in a crocked smile or a reduced smile. If you have too much gum to teeth ratio, speak to a dentist on options to contour your gums!

Oily Scalp

An oily scalp is caused by overactive oil glands in the scalp or infrequent washing of our hair. Excessive oil attracts dirt more easily, possibly causes dandruff, gives off an odour and sometimes causes folliculitis (infection of hair follicles). 

Botulinum toxin reduces oil production on the scalp like how sweat production can be reduced on palms and underarms. When administered into the skin of the scalp, it reduces the activity of the oil glands dramatically.

  • How soon can you see results for oily scalp:  3-5 days
  • How long can it last:  Usually lasts 6-8 months.
  • Units required for oily scalps:  The amount of botulinum toxin needed usually ranges between 35-60 units for the whole scalp.


As there is minimal muscle on the scalp, side effects for botulinum toxin scalp injections are minimal. Administered to the scalp and the back of the neck, tension headaches/migraines can be reduced as muscles responsible are relaxed. Compared to the underarms and palms, the procedure is slightly more uncomfortable as numbing cream application is more difficult. 

 Note: One should still see a dedicated hair medical professional for a proper diagnosis and understanding of scalp issues.

There are many more different functions for botulinum toxin, both aesthetic and medical! Botulinum toxin treatment to the chin area to relax a wrinkled chin can sometimes provide a lengthened, v-shaped chin effect.

All things are poisons and there is nothing that is harmless, the dose alone decides that something is no poison.

As the quote above, the right dosage makes it either a remedy or a poison – water is a thirst quencher, but even drinking too much water may prove to be fatal to a human.
The latest incident surrounding an aesthetic clinic and supposed botox treatment has garnered Mizu Aesthetic much attention, due to our prominence in the Marina Bay area and that the said clinic was confusingly reported to be located in the same area.
Therefore, we would like to clarify that Mizu is not the said clinic in the reports, and at the same time, seek the professional opinion of our Medical Director, Dr YZ Tan – a trainer in Botulinum Toxin injections, to shed the light on botulinum toxin, and what it can potentially do so as to clear up the stigma surrounding it.

What is Botulinum Toxin?

Botulinum toxin is a neurotoxin produced by the bacterium Clostridium botulinum, and there are botulinum types A – H – the most commonly used types for commercial/medical usage are types A & B [1]. It is in fact, a lethal toxin – but in minute doses, as with certain poisons, can be beneficial in more ways than one.

Are There Other Versions of Botulinum Toxin in the Market?

There are a few approved brands of botulinum toxin only for use in medical clinics in Singapore and by a licensed, trained doctor. At the same time, some brands are non-approved and non-FDA-cleared.

It is important to know what is being used for your face – if you have doubts, enquire with the clinic about the brand of botulinum toxin(s) available prior to the appointment.

What is Botulinum Toxin Commonly Used for?

A botulinum toxin treatment is a commonly done procedure – an estimated 3 million dollars worth of botulinum toxin procedures or more are done per year, worldwide [2]. Despite that, the knowledge as to how it works isn’t as widely understood as it should be!

As a neurotoxin, botulinum toxin, in layman terms, causes relaxation/temporary paralysis of the muscles. This can be applied in both cosmetic and therapeutic treatment usage, and the effects of botulinum toxin treatments usually last anywhere between 3 – 9 months (depending on the area treated), and may be repeated for maintenance.

“Botulinum toxin blocks presynaptic acetylcholine release, thus preventing the nerve impulses responsible for muscle contraction, and can be used to treat all wrinkles that are the result of normal facial movement. [3]“

Are You Too Young for Botulinum Toxin Treatments?

As we read the comments online, we saw this one comment that is a commonly known perception by some members of the public – at what age should one start to consider botulinum treatments?

We age every day.

While aging does play a part in our skin becoming thinner and more prone to wrinkling, these frown/laugh/smile lines are formed due to repeated movements, and botulinum toxin treatments serve to prevent or lessen these movements by relaxing the muscles involved.

A younger person could have the bad habit of unconsciously and excessively frowning and developing these lines earlier in life, too! Botulinum toxin treatments are typically used as a preventive measure to prevent these movement lines from becoming permanent and static. So yes, while botulinum toxin treatments are typically done for adults above a certain age threshold, you do not need to be of a certain age to use it.

Botulinum toxin treatments should not lead to a “frozen look” when the right amount is used – this requires a trained eye and hands – a licensed doctor who is able to gauge the amount needed.

Can Botulinum Toxin Treatments Cause Cardiac Arrest?

Quoting Dr YZ Tan, “There are no known cases of botulinum toxin deaths directly caused by heart failure. However, an allergic reaction, which is very rare, can result in shock and indirectly result in heart failure when not caught and treated in time.”

Are Botulinum Toxin Injections Lethal?

“Botulinum toxin procedures, when administered properly, is considered safe.”

Botulinum toxin is generally considered to have a wide safety margin [4]. Side effects from cosmetic use are generally non-fatal and usually result from the unintended paralysis of certain facial muscles. These can include partial face paralysis, muscle weakness, trouble swallowing, headaches, and flu-like symptoms. As the effects of the treatment wear off, one will regain muscle strength and movement, and these symptoms will go away.

However, as with any procedure, there are always risks involved, and while there have been claims of 16 deaths reported to be in relation to receiving botulinum toxin procedures, these cases are extremely rare, and if any, they are typically and possibly due to complication(s) arising from:

  • Off-label usage of the drug [5]
  • Mixing the wrong dosage [6]
  • Allergic reactions to the drug [7]

“Side effects from therapeutic use can be much more varied depending on the location of treatment and the dose of toxin used. In general, side effects from therapeutic use can be more serious than those that arise during cosmetic use. These can arise from paralysis of critical muscle groups and can include arrhythmia, heart attack, and in some cases seizures, respiratory arrest, and death.” – Wikipedia

“Allergan spokeswoman Caroline Van Hove said children with cerebral palsy receive far larger doses administered into their leg muscles than the doses given adults seeking wrinkle care.” – NBC News. [8]”

What Are the Signs That I Am Suffering From a Side Effect of Botulinum Toxin?

Common and negligible side effects of botulinum toxin procedure would include an achey/sore sensation at the site of treatment and possible mild bruising.

However, one should seek medical attention should they experience symptoms such as difficulty in swallowing or breathing, unclear speech, extreme muscle weakness, or difficulty holding up their head.

How Can One Ensure That Botulinum Toxin Treatments Are Safe?

In Singapore, only licensed doctors are allowed to perform botulinum toxin procedures. Make sure that you visit an experienced doctor.

Ask the clinic what it has or uses – At Mizu, we only use approved brands of botulinum toxin.

One of the most hyped up topic in recent years has got to be “anti-ageing”, a term widely favoured by brands marketing various products and services as let’s face it, who doesn’t want to be forever young?

Ultherapy has been one of the very same treatments that have been consistently lumped in with this catchphrase so we decided to shed some light on what exactly Ultherapy is all about and how it can help fight the signs of ageing.

What is Ultherapy?

Ultherapy is a U.S. FDA-approved treatment that uses micro-focused ultrasound to tighten and lift the skin. The heat generated from the ultrasound pulses creates specific coagulation zones at different depths within the skin. This targeted heating allows the treatment to achieve the desired firming or tightening effects safely without harming the surface of the skin.

In addition, Ultherapy remains uniquely positioned amongst the various energy devices due to the ability for ultrasound visualisation of the treated tissues in real-time to ensure the maximal safety and efficacy of each shot delivered.

So how can Ultherapy help reduce the signs of ageing?

As we age, several things happen:

Collagen and elastin producion decreases

This leads to increased skin laxity, sagging of the skin as well as a reduction in the overall firmness of the skin.

Weakening and stretching of the SMAS (Superficial Musculo-Aponeurotic System) layer

This important layer within our skin is responsible for providing support and definition for our facial features. As it weakens, it leads to an overall loss of definition and structure to our face.

Displacement of superficial fat pads

This leads to the deepening of nasolabial folds and increased jowling as the fat pads on our face migrate downwards with the general weakening of the support structures of the skin.

With each Ultherapy treatment, there is immediate contraction and denaturation of collagen due to the heat generated from each ultrasound pulse. How is Ultherapy administered?

Before the procedure, there will be a detailed consultation to ensure that Ultherapy is the most appropriate treatment to address your concerns. 

If suitable, your face is gently cleaned and a topical numbing cream is applied for roughly 30 minutes to ensure the procedure is as comfortable as possible. 

Unfortunately, as with most procedures, there will be some heat and discomfort due to the energy used but the phrase to remember is “no pain, no gain”! That being said, we also offer local anaesthesia at Mizu, which allows the procedure to be a lot more manageable and most patients tolerate it just fine.

Post-procedure, we cleanse the face once again before applying moisturiser and sunscreen. Optimal results usually start to show around three months and a complimentary review is scheduled around that time to follow up on the results and plan for further treatments as necessary.

Why is Collagen so important?

HIFU treatments, with all the rage in recent years, have become de rigueur for face lifting. For the uninitiated, HIFU, short for High-Intensity Focused Ultrasound, delivers the power of ultrasound energy deep into your skin. It targets the dermis and SMAS layers to stimulate collagen reproduction for a lifting and tightening effect. Because the SMAS layer is where surgeons target during a surgical facelift, HIFU has earned itself a reputation of being a non-surgical facelift.

Ultrasound energy-based treatments have been around for many years. In today’s market, there are a plethora of devices and establishments offering similar services at varying prices with the same touted benefits as a HIFU treatment. Go for a medical-grade HIFU machine

There are many different brands of machines for HIFU treatments in the market. In Singapore, only medical clinics offer HSA-approved medical-grade ultrasound procedures performed by a doctor. The Ultraformer III, also known as “Shurink”, is the most popular HIFU machine from South Korea. Other than the usual 1.5mm, 3mm and 4.5mm handpieces like with Ulthera, the Ultraformer III also comes with additional handpieces to target more areas. The 2mm handpiece is designed for the eye and brow, while the 6mm handpiece targets double chin, arms and bra areas. Which is the right HIFU for you?

Both Ulthera and Ultraformer III deliver similarly good results when done properly. While Ulthera remains the pioneer brand with a global following, the Ultraformer III is a slightly faster procedure and is more comfortable, although it utilises more shots than Ulthera which is considered slightly stronger in power.

The amount of power delivered however, ultimately depends on the energy settings and number of shots used. During your consultation, your doctor will be able to advise on the best course of treatment for you.

HIFU requires expert planning

There is no one-size-fits-all regime for HIFU treatments. Your doctor should conduct a thorough assessment of your face and concerns before customising a treatment plan for you according to your face shape and desired outcomes. Different parts of your face will require different power settings, treatment depths and number of shots to be administered for optimum results. There should never be a fixed number of shots for everyone. An individual’s treatment plan can differ greatly from another.

Adequate number of shots used

The treatment area and concerns in these target regions will determine the number of shots required. It is essential that your doctor uses sufficient shots to achieve the best results. Areas with heavier volume or more fats will require more shots as compared to areas with less volume. 

As a rule of thumb, a full face treatment requires an average of 800 to 900 shots. Every face is different and HIFU should not be a fixed protocol for everyone.

The power should be strong enough

HIFU generates a high amount of heat of up to 72 degree celsius under the skin. This damages fat cells and stimulates collagen reproduction. The power of the ultrasound energy delivered therefore determines the degree of collagen reproduction and volume of fat reduction.
For volume reduction around double chin or jowls, strong power settings are required to generate heat of 68 to 72 degree celsius. This ensures that fat cells are damaged to achieve the desired lifting and slimming effect.

A proper procedure at power settings that deliver good results will inevitably be uncomfortable to some degree. A painless or low-powered HIFU treatment does not generate enough energy to deliver efficient results. Simply put – no pain, no gain. 

Pain management optimises your results

Pain management is essential for achieving significant results without compromising on comfort. This power and pain management and how we do it at Mizu is a very important part of the procedure.
While we apply topical numbing cream for all our patients and sometimes offer oral painkillers, we also use local anaesthesia to perform a nerve block to manage discomfort.
A nerve block is the injection of local anaesthesia to numb the face, like how a dentist will inject local anaesthesia to numb the gums before a tooth extraction. A nerve block greatly reduces any possible pain, which would allow us to perform the HIFU at the optimum power setting for better results.

Are you paying for the right HIFU procedure?

HIFU stands for “High Intensity Focused Ultrasound” — it works by focusing ultrasound energy into heat energy. When focused on the skin layers, it remodels and stimulates collagen growth.

A proper HIFU not only stimulates collagen and lifts sagging skin. It also reduces  unwanted facial fat, especially around the lower face area, including double chin removal.

There is also the Fractionated Ultrasound (also named HIFU), commonly offered by medi-spas and facial spas. i.e. you don’t need a medical license to operate these machines. They are also the choice of ultrasound anti-aging device for certain clinics.

The main difference is the energy output. A proper HIFU generates temperatures under the skin to more than 70 degrees, while spa-grade ultrasound procedures only generates temperature of up to only 42 degrees.

Also take note that a proper HIFU’s handpiece moves on the face in a linear pattern, not  in a continuous circular motion. The handpiece is stationary when shots are performed to ensure that the focused ultrasound energy stays at one point. This properly targets the specific area and depth, so it should not be ‘spread’ around in a continuous motion.

An ultrasround anti-aging procedure that delivers good results is inevitably uncomfortable. A proper HIFU is painful at higher energy levels – even with numbing cream, some patients still feel the discomfort. Frequently Asked Questions

How long before I see the results?

A proper HIFU takes 3 month to see the best results – however, short term results might be visible due to the heat causing some tightening.

If it requires more than 1 session, it is NOT a proper HIFU.

Due to its high energy, the HIFU is done once every 1 – 1.5 years.

Spa-grade treatments usually require 3 or more sessions. Even with more sessions, results are not as significant as a proper HIFU due to its lower energy output.

The cost of the treatment(s) tells you most about the efficacy. A proper HIFU takes careful planning and execution, and prices are 2 – 3 times the cost of a single session of the spa-grade machines. 

However – spa-grade machines usually require 3-4 sessions for best results plus regular maintenance sessions after. The total works out to be the same amount or more!

At high energy levels, a properly done HIFU may cause some puffiness along the jawline/lower face. There is little to no redness as it targets deep within the skin. Puffiness usually lasts 3 days on average –  soreness along the jawline can occur too.

The higher the energy setting, the longer the puffiness lasts. Treatments that claim zero downtime = not performed at the optimal energy settings, which gives unsatisfactory results.

The Ulthera is a brand of HIFU that originates from America. Ulthera, or Ultherapy is the first ultrasound anti-aging device in the market, and has a bit global following. It is also the first HIFU machine to introduce skin-imaging via Ultrasound. This allows practitioners to assess the depth where thermal ultrasound energy focuses on.
